Any Middle and High School Student is welcome to make an appointment for our School & Sports Physical Night on Thursday, June 20th. Licensed Athletic Trainers, Physical Therapists, Registered Nurses, and Physicians will be on-site in the Regina Cafeteria from 4:30-7:00 PM conducting the exams, which will be valid for the 2024-2025 school year. Exams are $25 - cash or check made payable to Team Rehabilitation. 

Parents are not required to attend; however, a valid parent signature must be on the form before the exam can begin. MHSAA physical forms will be provided but can also be downloaded and printed from www.mhsaa.com. 

As a reminder - all incoming freshmen are required to have a physical uploaded to Final Forms before the first day of school. All student-athletes are required to have a physical uploaded prior to trying out for a sport. A valid physical is one dated on or after April 15, 2024.
